In short

A citation gives a reviewer somewhere to begin. This log records whether the source actually supports the statement being used, with enough location detail for another reviewer to repeat the check.

Download the blank source review log. Use one row per consequential statement or required field, not one blanket “reviewed” row for a complete proposal.

Perform the check

  • Identify the output: Record an internal output reference and version. Use approved identifiers, not unnecessary personal information.
  • Find the source: Identify the document version, page, section, table, or other precise location.
  • Compare the meaning: Check the full relevant passage and qualifications, not only the highlighted sentence or a generated excerpt.
  • Record the result: Use supported, contradicted, omitted, ambiguous, or source unavailable. Define any local additions consistently.
  • Resolve or hold: Record the correction, unresolved question, responsible person, and disposition. A checked citation is not automatically an approved client communication.

NIST's generative AI profile includes verifying sources and citations during evaluation, while insurance professional guidance recommends treating AI output as preliminary and checking insurance summaries and comparisons against the actual source terms (NIST GenAI Profile; Independent Agent).

A synthetic example

Suppose an invented output says “Deductible: $1,000,” but the relevant synthetic source says “$1,000 per occurrence, except wind/hail subject to the separate schedule.” The number matches; the unqualified statement does not preserve the exception.

Record the missing qualification, identify the applicable schedule, and hold the statement until the reviewer resolves how it should be presented. This example illustrates verification, not a coverage interpretation or a claim about any actual form.

Keep evidence usable

Store the log with controlled references to the actual reviewed version. If a replacement source arrives, reopen affected rows instead of allowing the old sign-off to appear current.

Read A source link is a starting point, not proof. Use human-review guidance to assign who can resolve a conflict and who can release the final work.
